What do I need to open a SSIA?
1. Complete our standard SSIA application form/direct debit mandate by applying online using our Internet

This should then be printed, signed and posted along with all the other items as outlined below.

2. Provide a copy of an official document which contains your Personal Public Service Number (PPSN),
formerly known as the RSI number. You will find your PPSN on a;
- payslip
- social welfare payment book
- tax free allowance form/notice of tax credits document
- P60 / P45
3. If you are not an existing Anglo Irish Bank customer, to satisfy Money Laundering legislation requirements,
please include;
- Copy of Passport/Driving Licence
- Original of a recent utility bill
(e.g. gas, electricity, etc.)
4. Complete and sign the Government Declaration Form
(The full SSIA Application in PDF format includes the Government Declaration form - if you have already downloaded the full application form
as outlined above, you do not need to print this Declaration form separately)
